PAYMENT INFORMATION

Payment for all services including New Consultations and Follow Up Care (Office Visit / Virtual Office Visit) is due at the time your appointment is scheduled.  24 Hours advance notice is required if you wish to cancel or change your appointment without penalty.

Patients requesting a follow up visit may do so at ATMrequest.com – You will be asked to pick a preferred Day of the Week and Time Slot, then we will call you to confirm the actual appointment time.

If you reside in a country that is not yet handled by our online system, or a medication you take is not listed.  Please contact PatientCare@ATMAntiAging.com

If you need a coded receipt for insurance and your original DID NOT have codes, then you may request one free of charge by emailing PatientCare@ATMAntiAging.com otherwise duplicate receipts may be requested at ATMRequest.com for a $20 charge.  Please note that if you paid for your request online, receipts can be viewed by logging in, clicking My Account, and then Orders.


ABOUT LAB TESTING

Lab Test Results – Dr. Crisler must review lab results with you at a follow up visit before we are authorized to release them.  Requests for labs not yet reviewed by the Doctor will not be honored and no refund will be offered as well as a non compliance fee being charged.  These results are provided via email, and can be requested at ATMRequest.com for a $5 fee.

Have your results come in?  When we receive the results we will notify you that it is time to schedule your follow up appointment.  If it has been MORE THAN THREE WEEKS since you did your testing, the lab may have made an error.  Contact the lab and ask that they re-fax your results to our office at 815-301-2716.  If you have trouble dealing with your lab or had blood drawn at our office, then please email PatientCare@ATMAntiAging.com.  If it has been LESS THAN THREE WEEKS, DO NOT call or email the Lab or our office regarding results.

Frequency of lab work will depend on your particular case. We require lab work twice annually for you to remain an active patient. If the doctor has scheduled lab testing, you will receive pre-lab instructions via email once the requisition has been submitted. The email will be your queue to proceed to the lab as directed. If you were expecting tests to be ordered and it has been more than a week from the expected date, you are close to the six month mark since your last testing, or if the doctor asked you to contact us when you are ready for your testing, email PatientCare@ATMAntiAging.com.

Specific Lab Tests – Dr. Crisler will order the tests he feels is necessary for you.  If you are paying out of pocket for lab testing, or would like any special assays run, you will need to discuss this during your office visit.  The doctor will not consider any modifications to lab orders requested at other times.


DURING AND AFTER YOUR OFFICE VISIT


You will have about 15 minutes with the doctor.  It is important that you have organized any questions you may have - see pricing scale for number of issues, additional issues can be purchased when scheduling if needed.  Also, HAVE A PEN AND PAPER handy as the doctor may give you important information such as medication names and dosages that you will be responsible to remember.  If you did not pay attention to the Doctor during your visit and a Prescribed Medication List with Dosages, you may request one at ATMRequest.com for a $20 fee.

We have implemented a new request system at ATMRequest.com to handle all patient requests.  This includes scheduling and other administrative tasks, prescription call-ins, medication requests, and more!  You will no longer need to check out with front office personnel after your visit.  If you need medications simply request those that the doctor authorized during your visit through the online system at ATMRequest.com.

If you wish to submit a request in person at our office, via fax, phone, or email there will be a $20 processing fee added automatically.


INSURANCE

To streamline our ability to provide excellent medical care our office does not do any insurance billing.  If you have insurance that covers any services or medications, we will provide a coded receipt for you to submit for reimbursement.  If needed you can also request a letter of medical necessity or prior authorization by selecting Letter Writing on the ATMRequest.com system.


PATIENT STATUS – STAYING ACTIVE

To remain an active patient, you must have an office visit, then we must review your lab testing, at least two times per year.

CONTACTING OUR OFFICE

Inquires are addressed in the order received.  Emails will receive a faster response than phone messages.  Our email address is PatientCare@ATMAntiAging.com.  Patients are not to leave multiple messages regarding the same issue, as it will reset your request date and put you back at the end of the queue, which will delay your response.  It may take up to 2 business days for us to respond to your inquiry.  If you have not heard from us in this timeframe, please try to contact us again as we may not have received your original inquiry.
